[00:08] Clinician: How is she sleeping these days?
[00:11] Parent: Ten to eleven hours most nights. She still fights bedtime a little.
[00:40] Clinician: And eating? Any concerns there?
[00:43] Parent: She's picky. Mostly pasta and fruit, and she drinks milk with every meal.
[01:20] Clinician: Her speech is clear and she can hop on one foot. Let's keep milk to about two cups a day so she's hungrier at meals. She's due for DTaP, polio, MMR and chickenpox today.
[01:38] Parent: That's fine. Let's do all four today.

How the clinic uses Charted.
Kelden Pediatrics has 14 clinicians across two offices and sees newborns through teenagers. A well-child visit fits milestones, screenings, growth and vaccines into 20 minutes, and most of the history comes from a parent talking fast.
Clinicians record from a phone on the counter once the parent, and an older child or teen, accepts the consent prompt. Charted drafts the note in the group's own well-child template for that age and lists the vaccines due and any screening results discussed. Anything not said aloud, such as the physical exam, is flagged for the clinician to complete.
During the pilot, median time to a signed well-child note was 3 minutes, and clinicians spent about 2 fewer hours a week charting in the evening. The practice now records every well-child and sick visit.
