Signed notes land in the chart you already use.
Charted connects to your EHR through standard health-data APIs. Each part of the signed note goes to the field where it belongs, and nothing is sent until the clinician signs.
Every part of the note has a place in the chart.
A typical mapping. We set up yours with your team during the pilot.

How a connection is set up.
1
Tell us your EHR
We confirm which API your EHR offers and what access your IT team needs to grant.
2
Map your template
We match each section of your note template to a chart field and test it with sample patients.
3
Sign and send
Pilot clinicians sign their first notes, and we check that each one landed in the right field before you roll out.
Integration questions.
Which EHRs does Charted connect to?
Any EHR with a standard health-data API, which covers most systems clinics use today. If yours doesn't have one, signed notes can go out as documents your staff file in the chart.
Does Charted need write access to our EHR?
Only to the fields you choose to map, and only for signed notes. Charted never changes a note after the clinician signs it.
What happens if the EHR is unavailable?
Signed notes wait in a queue and go out when the connection is back. Clinicians can see which notes are still waiting.
Who sets up the connection?
Our team does, with your IT contact. Most connections are ready within the 14-day pilot.
