How Charted started.
In the winter of 2020, emergency physician Nadia Brennan kept a tally: for every hour with patients, she spent 40 minutes on notes, most of them after her shift. Her friend Tomas Ekberg had spent ten years building speech recognition for call centers, where software already listened and summarized. They started Charted in 2021 with one rule: a clinician signs every note.
The first version ran in one urgent care clinic in Queens. Its drafts were rough, and the clinicians there marked up every one. Five years later, 2,400+ clinicians in 180 practices use Charted, and practicing clinicians still review every note template before it ships.
