What brings you in today?

Headaches for three days, and my stomach hurts.

The chart shouldn't follow you home.

Charting after clinic

Notes left over from clinic get finished at home at night. Clinicians call it pajama time.

The draft is ready a minute after the visit ends, so you can sign it before the next patient.

Built for how clinicians write.

Notes follow the structure your team charts in, and each suggestion links to the transcript line behind it.

Patient history summaries

Open each visit with a short summary of past notes, recent labs, and current medications.

Visit transcription and note drafting

Once the patient agrees, Charted records the visit and drafts the note in your template while you talk.

Diagnostic suggestions for review

Possible diagnoses and open questions come with the transcript line behind them. You decide what goes in the note.

From a 12-week pilot with 40 clinicians in family medicine and pediatrics.

Made for 30+ specialties.

Two clinicians reviewing patient notes on a tablet

Charted writes in the format your specialty already uses: SOAP notes in family medicine, DAP notes in behavioral health, and ED notes with medical decision-making. The clinician who saw the patient reviews and signs each note.

From the clinicians who sign the notes.

“I used to finish charts at the kitchen table after my kids were asleep. Now I sign the last note before I leave the building.”

Questions clinicians ask.

Do patients have to agree to being recorded?

Yes. Charted shows a consent prompt before every visit and saves the patient's answer in the note. Patients can ask you to pause or stop at any time, and their care doesn't change.

Which EHRs does Charted work with?

Charted connects to common EHRs through standard health-data APIs. If yours isn't connected yet, we set it up during your pilot. Until then, you can paste each section of the note into the chart.

Does Charted make clinical decisions?

No. Charted drafts documentation and suggests codes and possible diagnoses for you to review. Nothing reaches the chart until a licensed clinician reviews and signs the note.

How do I check that a note is accurate?

The transcript sits next to the draft. Tap any sentence to see the moment in the visit it came from. Charted also flags phrases it couldn't hear clearly, so you know where to look first.

What happens to the audio after the visit?

Audio is encrypted on the device and deleted once you sign the note, unless your organization sets a different retention period. The transcript and the signed note stay in your account.

Can I use Charted with patients who speak another language?

Yes. Charted transcribes visits in more than 20 languages and drafts the note in English. Patient instructions can be written in the patient's language.

Can we use our own note templates?

Yes. Start from templates for 30+ specialties, or send us the format your clinic uses and we'll set it up during the pilot. Admins can share templates across the practice.

How does the free pilot work?

The pilot runs for 14 days with unlimited visits, and you don't need a credit card. We sign a BAA before the first recording and help set up your templates and EHR connection. If you don't continue, export your notes and we delete the rest.
