[00:14] Clinician: How have your home blood pressure readings been since last time?
[00:19] Patient: Mostly around 140 over 90. I take the pill every morning, I set a reminder on my phone.
[00:31] Clinician: Any headaches, chest pain or shortness of breath?
[00:35] Patient: No, none of that.
[01:02] Clinician: Today you're at 146 over 92. You're taking it every day and still above goal, so for your hypertension let's go up to 20 milligrams of lisinopril and check your kidney function and potassium in two weeks.
[01:15] Patient: Okay. I'll keep logging my readings at home.

How the clinic uses Charted.
The nine clinicians at Ostrin Family Medicine see 20 to 24 patients a day, many with two or three chronic conditions. Before Charted, most notes were finished at home, and dose changes agreed in the room sometimes didn't make it into the plan.
Now the patient accepts the consent prompt, and the clinician starts recording and puts the phone face down. When the visit ends, the SOAP draft is ready, with medication changes, ordered labs and follow-up in the plan. Suggested ICD-10 codes sit beside the note with the sentence behind each one.
Over a 12-week pilot, median time to a signed note fell from 15 minutes to 4, and 96% of notes were signed the same day. The clinic kept its own SOAP template, section names included, and now records every scheduled visit.
