Terms of Service
Last updated October 1, 2026
This page is sample text from the Charted template. Replace it with your own terms, reviewed by your counsel, before you publish.
These terms apply to clinics and the clinicians and staff who use Charted. By creating an account or using the service, your organization agrees to them. If you sign an order form with us, its terms take priority where the two differ.
Accounts
Your organization chooses who gets access and is responsible for the accounts it creates. Keep sign-in details private, turn on single sign-on where it is available, and tell us right away if you think an account has been misused.
Clinical responsibility
Charted drafts documentation. It does not diagnose, treat or make clinical decisions. A licensed clinician reviews, edits and signs every note, and the signed note is the clinician's record.
Suggested codes are suggestions. Your organization remains responsible for the accuracy of what it documents and bills.
Acceptable use
Record a visit only after the patient agrees, and follow the recording-consent laws that apply where you practice. Do not use Charted to record anyone without consent, to break the law, or to test or copy the service.
We may suspend an account that puts patients, other customers or the service at risk, and we will tell you why.
Fees
Plans are priced per clinician, as listed on the pricing page or in your order form. Fees are billed monthly or yearly in advance and are not refundable for a partial period, except where the law says otherwise. We will give you 30 days' notice before a price change takes effect.
Termination
You can cancel at any time, and access continues until the end of the period you have paid for. We can end these terms if your organization materially breaches them and does not fix the breach within 30 days of our notice. After termination, we return or delete your data as described in our Privacy Policy.
Liability
We provide the service with care and keep it secure, but we cannot promise it will be free of errors or interruptions. To the extent the law allows, neither party is liable for indirect or consequential losses, and each party's total liability is limited to the fees paid in the 12 months before the claim.
Changes and contact
We will tell you by email at least 30 days before a material change to these terms. Questions can go to legal@charted.example.
