Your clinician uses Charted to write visit notes.
Charted listens to your visit and writes a first draft of the notes. Your clinician reads the draft, fixes anything that's wrong and signs it.

What happens at your visit.
What's recorded and who sees it.
You can say no.
You can say no or ask to pause at any time. Your care doesn't change. Your clinician will take notes the usual way.
Questions patients ask.
Is someone listening to my visit live?
No. No person listens during your visit. A computer turns the recording into text for your clinician, who reviews the draft and signs the note.
Is my visit used to train AI?
No. Recordings, transcripts and notes are never used to train AI.
How long is the recording kept?
Usually until your clinician signs the note, often the same day. Some clinics keep it for up to 30 days. Then it's deleted.
Can I see my note?
Yes. Ask your clinic for a copy, or look for it in your patient portal if your clinic has one.
Who do I contact with questions?
Start with your clinic. You can also email our privacy team. We reply within 5 business days.
